These terms and conditions govern participation in the UA Program
1.1 - No Affiliation with USHA. The UA Program is independently developed and offered by Patriot Pixels LLC. It is not affiliated with, endorsed by, or officially connected to USHEALTH Advisors ("USHA") or any of its parent or partner companies.
1.2 - Clarification of Audience. The term "USHA agents" is used solely for identification purposes to describe the target audience of the program. Use of this term is for descriptive purposes only and does not imply any formal affiliation, partnership, or endorsement.
1.3 - No Representation. Patriot Pixels does not claim to act on behalf of USHA, nor does participation in the UA Program suggest any official approval, sponsorship, or endorsement by USHA.
1.4 - Intellectual Property Respect. All trademarks, names, and logos of USHA or its affiliates remain the exclusive property of their respective owners. Patriot Pixels makes no claim to these assets.
1.5 - Client Acknowledgment. By participating in the UA Program, clients acknowledge and agree that they understand there is no formal relationship between Patriot Pixels LLC and USHA.
2.1 - USHA-Focused Eligibility. The UA Program is available exclusively to licensed U.S.-based insurance agents affiliated with or operating similarly to agents of USHEALTH Advisors ("USHA"), subject to approval.
2.2 - Qualification Process. All applications are subject to a manual review and qualification process. Patriot Pixels reserves the right to accept or reject any applicant at our sole discretion.
3.1 - Custom Website Design. Approved clients will receive a fully custom-designed, responsive website up to three (3) pages (e.g., Home, About, Contact), built using the tools, platforms, and resources deemed most suitable by Patriot Pixels for the project.
3.2 - Standard Features Include:
Custom domain setup
Mobile responsiveness
Basic SEO setup (title tags, meta structure, etc.)
Custom contact form
Stock imagery as needed
Limited design revisions
3.3 - Exclusions. The UA Program does not include e-commerce, appointment booking, client portals, membership logins, custom integrations, or advanced features unless separately quoted and approved
4.1 - Hosting Subscription. Participation in the Program requires subscription to a Hosting Plan offered by Patriot Pixels.
4.2 - Minimum Commitment. A three (3) month minimum hosting term is required. Early cancellation will result in a fee equal to the difference between paid amounts and the 3-month minimum.
4.3 - Hosting Not Included in Website Offer. While the website design is provided at no cost, hosting is a paid service and the only required cost for participation.
5.1 - Timely Participation. Client agrees to provide all required content, images, brand materials, and platform access (if needed) in a timely manner.
5.2 - Feedback and Approvals. Client is responsible for reviewing drafts and providing approval or revision feedback within reasonable timeframes.
5.3 - Delays and Disqualification. Excessive delays, failure to respond, or unresponsiveness may result in removal from the Program without refund of hosting fees already paid.
6.1 - Designer IP. All code, styling, layout, and structural elements created by Patriot Pixels remain our intellectual property.
6.2 - Client License. Client receives a non-exclusive, revocable license to use the completed website while subscribed to an active hosting plan.
6.3 - Client Content. Client retains ownership of all text, logos, and media provided to Patriot Pixels and grants us a royalty-free license to use said content for building and promoting their website.
7.1 - Optional Features. Additional services such as instant quote forms, custom logo design, lead capture landing pages, or other premium features are available as paid add-ons.
7.2 - Payment Terms. Add-ons may be billed as a one-time fee or monthly installment, as listed on the UA Program landing page or official pricing sheet.
8.1 - Right to Change Terms. Patriot Pixels reserves the right to update, modify, suspend, or terminate the UA Program or its terms at any time without prior notice.
8.2 - Continued Participation. Continued use of the UA Program constitutes acceptance of the most current version of this Agreement.
